Darwin, some people can't do it. Or so I gather. I did this regularly when I was in my early 20s and never had problems with urinary tract infection. It never occured to me to use a condom, but then I always insert my penis soft, because it goes in easier, and so a condom wouldn't have stayed in place.

While there are those whose urethras are practically impervious to contamination by enteric bacteria, there are others who have very little resistance to it. (I'm certainly of the latter). If you've had any kind of history of prostatatis or urinary tract infection, I would strongly recommend against this practice or suggest the use of a condom.
