Is there an advantage to using a ring around the base (under sac) as opposed to a ring on the shaft alone.   I'm 65 yrs old with E. D. for several years and just purchased a pump with limited success but it will not maintain.

As for sizing, I used a string and measured top to bottom of my sack. Perhaps my boys are to big or it's excess lower belly fat, but the circumference is 9" which equates to 2.86" diameter.  The largest I can find is 2.5".  I'm also concerned about difficulty putting it on.  My boys are sensitive and I want to avoid any pain.

As far as keeping blood in the penile shaft to maintain an erection, I don't think there is any advantage, it may even be less effective than a constrictive band on the shaft alone. However, a ring which surrounds both the penis and the scrotal sac may decrease the onset of the ejaculatory reflex by restricting the action of the cremaster muscles on the testicles, thus, prolonging sexual activity.

The simple solution here, to avoid any pain and when using an Aneros massager, is dispense with cock rings altogether. Having an erection is totally unnecessary to achieve prostate based orgasms. This is so true and important to internalize. In my late forties, when the strength of my arousal and erections became weak and weaker, I believed my horniness would fade away and I‘d become kind of asexual. @riningjoe, reflecting about your original post a technique just crossed my mind, our good fellow @harper, unhappily long time not seen here, once introduced me to.

He sometimes, when lying on his back, used his boxer shorts, of which he put one leg opening around his cock at its base, then pulled it downwards and back under his butt and tucked it underneath him. That boxer short method is similar to something I do: Since it gets cold around here, I'll often dress up for sessions and wear oversized pyjama pants with a button up fly so my cock is not touching anything. Sometimes when things heat up just having the fabric touching the base and balls is too much and I have to slip them off in a hurry because I get too stimulated.
