I've been using one over the past few months. It's a water one that I use in the bath (called a Bathmate). I've heard the air pumps can cause some deformation if not used right, but I have no proof of that. apparently the water one doesn't do that.
So far, with regular use, the water one has made me thicker, and a little bit longer. Even flaccid, I'm a bit bigger, and feel much more confident. Kinda turns me on as well 🙂 I'm not sure how permanent that will be, but I assume it's not permanent at all (or I'll need to use it regularly).

I think you'll like the vacuum pump...its an intense erection that you really can't experience any other way. It's fun to see just how much pressure you can take. Enjoy!

I've used a specialist air pump. It's generally recommended to go no higher than 5hg and stick to 10-15 minute session(s).
Which is why it's advisable to try and get a pump with a gauge, as it's the only real way to keep an eye on pressure. Any damage (ex. bursting veins) may not be indicated by pain. That's also why a transparent (clear) cylinder is also helpful, as you can keep an eye on the penis during the pump for any possible concerns.

Nice start for a BPH therapy session when a Super-T finish is the intent of the session. Fleshlight is optional. My Uro started me on pumping when I turned 70.

My Uro asks that his E.D. patients get firmly erect for 5 to 10 minutes every day. He hands out an inexpensive free pump and a 'suggestion / how-to' sheet, usually on the second visit. I keep mine in the shower, pump up and stay erect while I shave or trim my face and brush my back. A shot of shaving cream to lube the lower half of the cylinder makes for a quick seal and even erectiion.

If a new pump lacks a small hole, I drill one and have a handy patch of adhesive tape to cover the hole for pump-up cycles. I use three cycles: First to 50%, then erect for about 30 seconds to pull blood and lymph into the Cavernosa. Then release the vacuum. Second pump-up is to about 90%, held for a minute. Third pump-up goes a bit over 100% for a few seconds, then release to 95% and hold that until I'm ready to climb out of the shower. Scrub off the the shaving cream with some microbeaded body wash and, "get on with a nice day."
Occasionally I run a shot of dilute Clorox through the hose and bulb.

Agree on the long-term effect being girth rather than length

Is there any permanent changes in girth or length if one stops using a pump?

I think so. I've lost almost an inch in length compared to age 30-35. A Magnum size condom has about the same fit and feel as a 'regular' one did a couple of decades ago.

My pump arrives today. I may have to give t a quick test run. It's a basic one but I'll take it slow as you guys have indicated.

I've read reviews that people claim they've achieved permanent gains, but as a whole, you'd need to do actual penis-extending exercises to get long term effects going.
More so, I was advised by an online community recently that I have to 'decondition' from penis exercises for a few months as using a pump too early has likely toughened my dick up, making gains unlike.y

What kind of pump works right? I got one that is just a plastic tube with a connection on top for the pump (and valve) and it came with a small 'pussy' that fits in the open end. Pumping it does get my dick hard, but the pussy thing just keeps getting sucked in the tube more and more and it's not possible to stroke the thing. I cut the end off a 'headhoncho' and it fit in the tube and that sort of worked but same thing, the honcho keeps just going in the tube more. I'd like to be able to use this thing because it's the only way I can keep a hard on but it just won't let me stroke. Maybe that's not the object?
