Almost always for foreplay and it usually stays in throughout intercourse. We initially bought a Helix, just for this purpose, and stuck with that for about four months then 'downsized' to an SGX. I use a 'stiffer' anal lube (to minimize leakage) during foreplay and the Helix was an occasional nuisance when I'd reposition rapidly. The SGX is easier going on that count.
