This position has I think trumped all other personal positions from my past. I’ve always been a left side man myself. But this extra step has helped me enormously. Here are the steps. 
lie on left side with thighs roughly at right angles to torso. Both thighs together. Now, lower your thighs away from this right angle so that your knees are pointed together downwards, as much as is comfortably possible. 
do all this on Aless or with a device. 
for me this has a powerful effect that in many cases brings some super Os. I recommend it. 🙂

when repositioning from knees at right angles to torso, to dipping knees down I forgot to mention, keep your calf’s in place ie don’t straighten the legs out  it kind of reminds me of praying. The tops of thighs go from right angles to the belly to a more obtuse angle compared to torso. Sorry if I’m not clear. Ask I and I’ll explain any questions.

Not being a doctor or anything I suspect I’m my case the increase in sensitivity down there is increased pressure on device and or prostate.

Thank you, as this is interesting.  So, you appear to say two different things here:

You say "don't straighten the legs out", but you have to to get to the obtuse angle (>90 degrees, <180 degrees).

Pictures or a drawing would be great.  Sorry it is slightly confusing.

This whole “complex” of Aneros and lower-body muscles, is like discovering you have another useful limb.  Some moves you will use often.  Others you may forget about, and return to later.  Things move in all kinds of ways down there, to pleasure’s benefit.  “Try it, you’ll like it”.  Let curiosity and intuition be your guide.

Almost two years of success in, and I’m still finding on my own or learning from members, new things to try.

You use your limbs for a lifetime, or ought to!

Am I understanding correctly that the idea is to extend the hips from the right angle without changing the flexion of the knee? So that your legs hook behind you a bit and your knees point down

I think an explanatory drawing would be good. IDK if there’s a shortcut for doing that unless it’s a stick figure?

Here’s a simple stick figure representation. As see from above over a figure laying on his left (or right) side. Position 1 is starting position and 2 is the held position. My drawing (and suggestion) is only an idea. No need to force yourself or if not comfortable for you another position maybe more suitable.
