I've been mostly trying the laying flat on your back with both legs bent method, but was thinking that I should try other ways. I like the side position at the start of the session. It’s easy to insert the device, and a great position for the critical relaxation. I then transition to being on my back. Excellent question! You will find different positions provide different results! Standing with legs spaced apart and bent over can give me amazing results! But then so can lying on my stomach as well as on my back! You just have to experiment to see what

I too start on my side in the recommended first use position. Then a few Os in i get on “all fours” except with my knees spread apart and my upper body weight resting on my shoulders (with a pillow for comfort). For me it's left side, both knees drawn up like sitting, hugging a pillow. gets me going almost every time. Some times I can get things going on my right side to start off but most times if things are moving along I can flip to the right side and keep going. I've never really had any luck on my back.
